• How much of a discount did I earn?
o Each Barix Customer will receive one "point" for each friend or family
member who attends a consultation with Barix Clinics between
November 1, 2007 and March 15, 2008 and brings in the filled-out
Consult Certificate to the Group Practice.
1 point = 25% discount
2 points = 50% discount
3 points = 75% discount
4 points = 100% discount
• How will I receive my discount coupon?
o Coupons will be sent to Barix Customers by email. If we do not have an
email address, the coupons will be sent by mail.
• When will I receive my discount coupon?
o Coupons will be sent to Barix Customers 2-4 weeks after the promotion end
date, March 15, 2008.
• How do I use my discount coupon?
o Online: Visit our online store at http://www.barixclinicsstore.com/. Add the
item(s) you want to purchase to your cart. Type your coupon code in the
"Apply Coupon" field located in your shopping cart and click "Go!"
prior to checking out of the store.
o Phone Orders: When you call us at 1-800-282-0066 to place an order by
phone, have your coupon code ready and tell it to the phone operator.
o In Person: Print and bring your coupon to a Barix Clinics location in Michigan,
Ohio, or Pennsylvania. Present it to the receptionist when you are ready to
check out of the store.
• What can I purchase with my discount coupon?
o All items available in the Barix Clinics Store qualify for this promotion.
• Is there a limit on how much product I can purchase from the store?
o The discount coupon may be applied to any Barix Clinics Store purchase up
to $200 (before shipping).
• Can I use my discount coupon multiple times?
o Each coupon is specific to each patient and can only be used once.
• Can I use my discount coupon in person instead of online?
o Discount coupons can be used online, over the phone, or in person at Barix
Clinics locations in Michigan, Ohio, or Pennsylvania. Phone orders may
be placed by calling 1-800-282-0066.
• Do I need to use my coupon by a certain date?
o The discount coupon will expire December 31, 2008
